The Carnifex is part of a Space Marine Apothecary's equipment, used to administer "The Emperor's Peace" - i.e. euthanize - wounded Marines whose wounds are too grave to be healed. It is shaped like a pistol, the barrel of which is applied to the wounded Marine's temple. When it is triggered, it sends a metal piston hurtling through the skull and into the Marine's brain, killing him instantly and virtually painlessly.
